  • Large saucepan or wide skillet - At least 2-quart/2L capacity, heavy-bottomed preferred
  • Box grater or microplane - For finely grating Parmesan

Ingredients
  

  • 4 tablespoons unsalted butter - 57g
  • 4 cloves garlic - about 1.5 teaspoons, minced
  • 2 cups heavy cream - 480ml, also sold as heavy whipping cream
  • 1.5 cups Parmigiano-Reggiano - 150g, freshly grated, plus extra for serving
  • 0.5 teaspoon fine sea salt - plus more to taste
  • 0.25 teaspoon freshly ground black pepper
  • 0.125 teaspoon ground nutmeg - optional, but highly recommended

Instructions
 

  • Mince the garlic as finely as possible and grate the Parmigiano-Reggiano using a microplane or fine grater. Let the cheese rest at room temperature while you prepare everything else.
  • Set a large, heavy-bottomed saucepan over medium-low heat and melt the butter until it is fully melted and just beginning to foam lightly at the edges, about 1 to 2 minutes.
  • Add the minced garlic to the melted butter and cook, stirring constantly, for 60 to 90 seconds until fragrant and translucent. Do not let the garlic brown or it will turn bitter.
  • Pour the heavy cream into the pan slowly while stirring, then increase the heat to medium and bring the mixture to a gentle simmer. Cook for 3 to 4 minutes, stirring occasionally, until the cream thickens slightly.
  • Reduce the heat to low and add the grated Parmesan in two or three small batches, stirring or whisking vigorously after each addition until the cheese is fully melted and the sauce is smooth and glossy.
  • Season with salt, pepper, and nutmeg, then taste and adjust as needed. Toss with cooked pasta over very low heat, adding reserved pasta water a couple tablespoons at a time if needed to loosen the sauce. Serve immediately with extra Parmesan and black pepper.

Notes

  • Always use freshly grated Parmigiano-Reggiano, never pre-shredded cheese from a bag, as it contains anti-caking agents that prevent smooth melting and can create a gritty texture.
  • Keep the heat very low when adding the cheese. High heat causes the proteins in Parmesan to seize up and turn grainy instead of melting into a silky sauce.
  • Let the cheese come to room temperature before adding it to the sauce for smoother melting. Take it out of the refrigerator 15 to 20 minutes before you start cooking.
  • Save about 1/2 cup (120ml) of starchy pasta water before draining your pasta. The starch acts as a natural emulsifier that helps the sauce cling perfectly to the noodles.
  • This sauce thickens quickly as it cools. Serve it immediately for the best texture and silkiness.
  • Store leftover sauce in an airtight glass container in the refrigerator for up to 3 days. Do not freeze, as the emulsion will separate and become grainy when thawed.
  • To reheat, warm the sauce very gently over low heat on the stovetop with a splash of milk or cream, stirring continuously. Never let it boil.
  • Add a pinch of nutmeg for a subtle warmth and earthy depth that enhances the rich cream and Parmesan flavors without overpowering them.
  • For a lighter version, substitute half of the heavy cream with whole milk. The sauce will be thinner but still flavorful.
  • This sauce is naturally gluten-free, making it an excellent choice for gluten-sensitive pasta lovers.
